The Food and Drug Administration (FDA) has approved the use of lenacapavir, a twice-yearly injectable medication developed by Gilead Sciences, for preventing HIV. This approval marks a significant advancement in preventative therapies for HIV-1. Previously, the once-daily pill known as PrEP was a primary method for prevention but required consistent intake and access to pharmacies.
Lenacapavir had already received FDA approval in 2022 for treating adults with multidrug-resistant HIV. Clinical trials indicated its potential as a preventative medication, particularly benefiting those who struggle with adherence to daily pills or face difficulties accessing prescriptions regularly.
The new FDA approval extends lenacapavir’s use as a preventative measure for all adults and adolescents weighing at least 35kg who are at risk of acquiring HIV. Joe Eron, MD from UNC School of Medicine played a key role in the clinical trials that led to this development. “This is game changing development in our field,” said Eron. He believes that if deployed effectively, especially in rural areas, this medication could significantly impact the HIV epidemic across the United States.
In North Carolina alone, about 38,634 people live with HIV. The state ranks among the top ten nationwide in terms of HIV rates. Many affected individuals reside in rural areas where healthcare access is limited—a situation Eron refers to as “healthcare deserts.” This makes it challenging for people to obtain prescriptions discreetly.
Eron highlighted that young men of color who have sex with other men often avoid taking PrEP due to stigma and lack of access to healthcare services. “North Carolina is no exception,” he stated, emphasizing the importance of lenacapavir being administered privately just twice a year.
Two phase 3 multicenter trials were conducted to evaluate lenacapavir’s safety and efficacy: PURPOSE 1 focused on sexually-active women in Uganda and South Africa; PURPOSE 2 targeted cisgender men and gender-diverse individuals across several countries including Argentina and Thailand.
Results published in reputable journals demonstrated lenacapavir’s high efficacy—100% effectiveness was noted during PURPOSE 1 while PURPOSE 2 showed approximately 99.9% protection against infection among participants receiving lenacapavir compared to those on Truvada.
“Through the PURPOSE trials we found that lenacapavir is effective at preventing HIV infection from taking hold in the body,” said Eron noting also its safety profile without toxicity concerns reported during studies.
High adherence rates are crucial for success with such treatments since oral PrEP requires several days of consistent use before becoming fully effective whereas injection schedules can improve adherence significantly by reducing frequency requirements—making it particularly beneficial post-prison release or after opioid therapy when risks peak temporarily according Eron’s observations regarding population needs based upon past experiences dealing within these contexts professionally over time now finally seeing positive outcomes realized potentially possible here moving forward
